1. Choose Your Payment Plan

If you prefer to spread payments over time, you can enroll in our clinic’s Payment Plan Program, which uses Affirm as the financing provider.

2. Pay the Enrollment Fee

To join the program, there is a $75 Payment Plan Enrollment Fee. This fee:

Covers administrative setup and management of your payment plan
Is charged by our clinic (not by Affirm)
Is not a cost of credit
Is optional - you may always pay in full instead

3. Complete Your Plan Through Affirm

After enrollment, you’ll receive a link to complete your payment plan through Affirm. Affirm provides all credit terms, disclosures, and approvals directly.
